Hockey Coffs Coast Player De-Registration Form 2024 - for SENIOR Comp

This form is to be completed to remove players from the Hockey Coffs Coast registration team lists.
Completing this form does not automatically entitle the player to a refund.
No refunds for Hockey NSW or Hockey Australia fees will be given.
Refunds for Hockey Coffs Coast fees will be considered on the number of remaining games at the time of de-registration of the player and the circumstances of the de-registration.

This form can be used for de-registering multiple players for the SENIOR Comp.
